Now you can become a Horizon customer instantly by simply filling out a quick cash or credit account application form. Once you complete the form and return it to Horizon’s credit department, you’ll be a Horizon customer and Horizon will e-mail you all of its great product specials and information on training programs to give you the Edge You Need.

Just follow these simple steps to apply for a cash or credit account with Horizon:

Cash Account Application:
1. Download a PDF of the cash account application form in English or Spanish.
2. Fill out the form and print it.
3. Sign the form.
4. Fax the completed and signed form to Horizon's credit department at 480.705.4072 or by e-mail.

Credit Account Application:
1. Download a PDF of the credit account application form.
2. Fill out the form and print it. Make sure you fill out the W9 form as well (last page of the PDF).
3. Sign the form.
4. Fax the completed and signed form to Horizon's credit department at 480.705.4072 or by e-mail.
